Lip Enhancement procedures can be divided into two categories:

    1. Surgical Procedures
    2. Injectable Fillers
1. SURGICAL PROCEDURES can be of three types:
    A) Autologous Tissue : transfer from your own body

      a) Fat: Fat is abundant and easily available in the body. For lip augmentation, only a small amount is needed and can be easily harvested under local anesthesia. Micro-fat injection is the preferred technique in which very small droplets of fat are deposited in different planes of the lip with the help of 1.8 mm cannula. To a variable extent, some of the injected fat may get absorbed by the body and therefore repeat injections may be needed.

      b) Fascia: Fascia is a dense white soft connective tissue available as thin sheet in some parts of the body like outer thigh and can be harvested through tiny incisions with the help of special instrument called ‘Fascia Stripper‘

      c) Vein: Superficial veins are abundant,they can be speared and as they are just beneath the skin , therefore very easy to harvest through tiny cuts. Their walls are soft, full of collagen, surface is smooth and the width is adequate for the lip when slit opened. It is one of the ideal autologous soft tissue for lip augmentation but unfortunately less used.

The above procedures involve both harvesting the tissue and taking three tiny cuts; one in the midline of the lip and one at each corner of the mouth to insert the material.

    B) Natural Biological Material (An Allograft) :

      Collagen Sheet: A natural collagen sheet (AlloDerm) made from donated skin is inserted through tiny incisions inside the corners of the mouth. Once the ‘AlloDerm‘ implant is in place, it eventually becomes integrated with the natural tissues.

    C) Synthetic Biocompatible Material implantation :

      ‘Gore-Tex‘, ‘SoftForm‘ and ‘Soft ePTFE‘ are other synthetic implant material options.

      The procedure involves either harvesting the tissue from other parts of the body or taking tiny cuts on the lip to insert the material or both.

2. INJECTABLE FILLERS:

There are many types of dermal fillers that can be injected in the lips and around the mouth. They help in increasing the volume of the lips.

    A) Collagen: Collagen, once the most common dermal filler, is used less often today as liquid collagen is absorbed and repeat treatments are necessary to maintain the results.

    B) Autologen: Autologen is an injectable dermal implant material made from the patient's own skin.

    C) Dermalogen: Dermalogen is an injectable Human Tissue Matrix (HTM) procured from donor tissue.

    D) Hyaluronic Acid Fillers: The most common fillers used today are products that contain substances similar to hyaluronic acid. Hyaluronic acid is a natural substance found in the body. These types of dermal fillers are sometimes called ‘Hyaluronic Acid Fillers.‘ The newer options are safer and the effects last longer.

      a) Restylane :A crystal-clear injectable gel. The hyaluronic acid in ‘Restylane‘ is very close to that naturally found in the body.
      b) HylaForm : A crystal-clear injectable gel. The hyaluronic acid in ‘Hylaform‘ is similar to that naturally found in the body.

   
  Technique:
 

One injection is usually sufficient to produce the desired result. Proper placement of the injected material is important. Result is immediate which may be temporary, depending on the material used. Injections may need to be repeated periodically. Another add on choice is laser lip rejuvenation, which tightens the natural collagen and elastic tissue beneath the lips.

   
  Benefits:
 

  • Immediate return to normal activities (one-day down time if sedated) for nonsurgical procedures; depending on the procedure and materials used, recovery period is within a week for surgical procedures.
  • AlloDerm and Dermalogen do not require patients to provide donor tissue and are not rejected by the body, because they are human tissue. Usually they have a pliable, natural feel.
  • Gore-Tex implants are permanent, easily inserted and well tolerated by the body.
  • Temporary procedures like collagen or fat injection can be appealing to patients who want to experiment with a fuller-lipped look, but not necessarily commit to it.

   
  Other Considerations:
 

  • Allergic reactions can range from prolonged redness, swelling or itching, to firmness at the injection site. Other possible complications include bleeding, lip asymmetry, and migration and extrusion of the implant. Normal swelling and bruising lasts from three days to a week.
  • Patients with a reaction to the pre-treatment collagen skin test; who have a serious allergy history; or are allergic to lidocaine (a local anesthetic) should not consider lip augmentation. Treatment is sometimes postponed for patients with active inflammatory skin conditions or with infections.
  • Collagen injections have a short-lived effect (9-12 weeks); fat injections provide longer-lasting results, but are sometimes unpredictable as to the degree of improvement, can have lumping or scarring effects, and must be harvested from another part of the body.
  • Soft-form is a foreign body and may become infected or be rejected. While many techniques exist for lip augmentation, none are ideal.

   
  Hyaluronic Acid Fillers
 

The most common non surgical lip augmentation is done with Hyaluronic Acid Fillers. Advanced injection techniques allow for precise control and customizable results by varying the enhancement of the lip subzones.

Hyaluronic acid fillers can improve the appearance of your lips by adding:

  • Shape
  • Structure
  • Volume

Although they provide excellent results, the effect is temporary and typically lasts for about 6 months. After that further injections are needed to keep the volume up

Once injected, the gel in the filler supports and shapes the tissues of the lips.

  • Control over lip volume. The amount of substance injected can be controlled, so the doctor has better control over how much lip volume is created.

  • Bumps dissolve easily. Any lumps and bumps created by movement of the lips can be dissolved easily.

  • Less bruising. There may be less bruising and swelling compared to other dermal fillers.

  • Reasonably lasting results. The results are reasonably long-lasting, but not permanent.

  • Allergic reaction unlikely. Because hyaluronic acid fillers are made from substances similar to those found in the body, they are unlikely to cause an allergic reaction.

  • Restylane, Restylane-L
  • Perlane, Perlane-L
  • Juvederm Ultra, Juvederm Ultra XC
  • Juvederm Ultra Plus, Juvederm Ultra Plus XC
All of these products are injected the same way and give you similar results. They all contain lidocaine, a local anesthetic and if you are allergic to lidocaine, please tell your doctor before being treated.
